Keep the action words distinct
Making room concerns space. Surgical access and attachment, when proposed, are distinct from orthodontic movement. These are different contributions, not required stages for every tooth.

Read relationships without inventing an order
Three parts of a reading, not three treatment stages
The panels sort information by meaning. They do not prescribe a sequence, technique or provider arrangement.
Purpose and action
Concept map; no anatomical viewpoint
- Goal: what is the stated aim?
- Action: what does the proposal say toward that aim?
- Connection: why are those two statements linked in this note?
Responsibility and remaining detail
Parallel reading categories, not a timeline
- Role: who is named for each part?
- Coordination: which connections does the team explain?
- Unresolved item: what does this particular note still leave open?
The model is a reading map. Its equally weighted panels have no treatment arrows. A purpose answers 'toward what?' An action answers 'what is proposed?' A responsibility answers 'who explains or carries out that part?' An action list alone is not a schedule.
Suppose a fictional explanation says its goal is a place for the named tooth in the upper row and its proposal includes a space-related action. The interpretation is a relationship: the action is proposed toward that stated goal. It is not a conclusion that the action will achieve the goal, nor that the same action belongs in someone else's plan.
Attribute each role to the actual proposal
The distinction is useful even when a note is incomplete. If the goal is clear but the responsible person for one proposed part is not, the unresolved item is that responsibility. It does not make the entire explanation unknowable, and it does not authorize a guess about who will provide care.
| What the explanation supplies | What the reader can retain |
|---|---|
| An orthodontist names the overall goal | Keep that goal attached to the proposal, rather than assuming it from the equipment or a picture. |
| A separate clinician is named for one part | Keep that specific responsibility with the named role. A mention of a role does not establish a particular practice's staffing or arrangements. |
| The team will clarify how the parts connect | Clinical coordination remains a team responsibility. The family can ask for one coherent explanation without being asked to design the handoff. |
A review plan has its own purpose
Monitoring and intervention are individually considered approaches. The BOS canine leaflet describes different possibilities and explains that suitability varies. It does not guarantee that a buried canine can be moved. Its 2019 UK information is general education, not a local protocol.

Distinguish a discussed option from the stated proposal
In a fictional conversation, a clinician might discuss a review-only approach, then describe a different proposed course. A faithful summary records both: the option was considered, and the note identifies what is currently proposed. Listing an option does not mean it was selected or that the website recommends it.
The complete case makes this distinction concrete. It supplies an actual proposed course within the fiction and identifies the remaining individual matters, instead of leaving you with an empty checklist.
